This is the fifth and final session in a five-part webinar series hosted in partnership with Proactive LTC Consultants.
Person-centered, individualized care planning is paramount to ensure optimal patient outcomes. This course delves into the intricacies of Minimum Data Set (MDS) systems and explores the mechanisms that drive care plan effectiveness. Participants will gain an understanding of Care Area Triggers, Care Area Assessments, and how to leverage them to develop and implement robust care plans tailored to individual patient needs.
Learning Objectives
- Participants will develop proficiency in utilizing MDS data to assess patient conditions, risks, and potential complications.
- Participants will understand the significance of accurate and timely data collection.
- Participants will learn practical approaches to implement care plans derived, in part, from MDS assessments.
Course Content
- 15 min: Explore the fundamentals of Care Area Assessments, their purpose, and their role in care planning.
- 15 min: Interpret MDS data to identify patient needs, strengths, and areas requiring focused intervention.
- 15 min: Explore strategies for developing individualized comprehensive care plans.
- 15 min: Discuss interdisciplinary collaboration and practical approaches to implement care plans derived, in part, from MDS assessments.

NAB Approved Contact Hours & CEUs
Each session is eligible for one contact hour (NAB). Proactive Medical Review will award the contact hour for primary registrants who provide their NAB ID at the time of registration and attend the full session.
Each session is eligible for one CEU for licensed Indiana administrators and licensed Indiana social workers. LeadingAge Indiana will award the Indiana-specific CEU for each individual who signs the CEU Sign-In form returned to LAIN after a session. Signing the CEU Sign-in form attests that you have listened to the webinar in full.
